When the senior exit questionnaire is completed:

1. login to Axio Survey, survey.ksu.edu
2. select the CIS Senior Exit Questionnaire
3. find the latest offering and click the Report button
4. then, at the top, click the Export Results button
5. make sure all the boxes are checked, and select Next
6. save the file to your machine

7. go to https://www.cis.ksu.edu/advising/seniorexitquestionnaire

8. at the top, enter the password finalINPUT
9. at the bottom, select the current semester and year, and add the file you just downloaded
10. hit Upload

To access the data:

1. go to https://www.cis.ksu.edu/advising/seniorexitquestionnaire

2. at the top, enter the password finalINPUT
3. in the middle select the options desired as well as the semester
4. select Get Results
